Family Practice at Primary Health Medical Group

Primary Health Medical Group (PHMG) is an independent, predominantly primary care medical group with 17 clinics and more than 320,000 patient visits per year. At PHMG policy is set by physicians and decisions are made based on what the doctors believe will lead to the best patient care. Our clinics are based on a patient-centered medical home model that gives the family doctor the support and time that encourages trusting relationships with their patients. Patient and physician satisfaction is very high. Our clinics have both family practice and urgent care at the same site.

COVID Testing & Vaccine

COVID-19 Vaccine 

Individuals age 6 months and older can receive an updated 2024-2025 COVID vaccine at Primary Health, as long as they have not received another COVID vaccine in the past two months.

Vaccines are offered on a walk-in basis, and there is no cost with most insurance plans. Availability may vary due to demand at each location.

New urgent care patients are always welcome at Primary Health Medical Group. To determine if Family Medicine health care providers or Specialists are accepting new patients, please call their respective offices.

Family Doctors

Our family doctors, physician assistants, and nurse practitioners focus on building relationships with their patients for better health outcomes. You and your provider will create an individualized health plan to address your specific health care needs, from general wellness to the treatment and prevention of chronic illnesses.
