
Whether you have established a health care relationship with one of our family practice providers, or you're visiting a Primary Health Urgent Care for the first time, we strive to provide you with an excellence experience.
New Patients
New urgent care patients are always welcome at Primary Health Medical Group. To determine if family medicine providers or specialists are accepting new patients, please call their respective offices or complete the Find a Family Doctor form, and our staff will contact you.
In an effort to help us meet your medical needs and make your visit go smoothly, Primary Health Medical Group has compiled some basic information for our first-time patients. Please read below and then visit our Forms page.
Urgent Care Patients:
- Primary Health Medical Group Urgent Care clinics accept walk in appointments, or patients can book online ahead of time for a shorter wait.
- Please bring along picture identification (such as a driver’s license) for the person seeking medical attention. If the patient is a child, please make sure the parent, guardian, or person responsible for the minor has photo identification.
- If the injured or ill person is covered under a health insurance plan, please bring the insurance card with you. Primary Health Medical Group accepts over 2,000 different types of health insurance and we are happy to assist with filing your insurance claim after your visit.
- During your first visit, you may be asked questions about your health history so that the medical provider can better assess your condition.
- Payment is due at the time of service (this includes any co-pay amounts for those covered under health insurance).
Family Medicine, Pediatric and Specialist Patients:
- For your first visit, please arrive at least fifteen minutes prior to your scheduled appointment.
- Please bring picture identification, applicable health insurance information (card), and any completed paperwork that may have been sent to you by our staff prior to your appointment.
- During your first visit, you may be asked questions about your health history so that the medical provider can better assess your condition.
- Payment is due at the time of service (this includes any co-pay amounts for those covered under health insurance).
