Saltzer Health has provided the following options for receiving copies of your medical records.

  1. Call 1-855-821-0591;
  2. Send a written request to Centralized Release of Information, 500 Eldorado Blvd, Broomfield, CO, Building 4 ; OR
  3. Email saltzerARC@imail.org
