FAQ

When are commissions paid?
Commissions are paid the middle of the month for the previous month's group payments.

What is participation?
Your group must meet participation requirements before it is sent to our Underwriting department for final rates. 2 or more full-time employees must be eligible to participate in group health coverage.

What is contribution?
The employer must contribute at least 50% of the employee's premium (up to 100%) and they may contribute between 0-100% of the employee's dependents.

How are probationary period employees handled?
We need an application or waiver for all employees eligible for coverage within 63 of the effective date of coverage

Can the employer get one bill for medical, vision, dental and life insurance?
Yes, if the dental is non-voluntary (all employees who select medical, must be on dental also).

Does the signature/date on the application matter?
The signature and date on the application must be within 63 days of the effective date of coverage.

Where do I get a new ID card?
Call your Account Executive and they will request a new ID for the member on your behalf.

How do I check to see if a new employee application was processed for enrollment?
Call your Account Executive and they will check on the status of the enrollment on your behalf.
