Short Term Essential Policy Application
SHORT TERM ESSENTIAL POLICY is designed for people who require temporary medical coverage and who are healthy. SHORT TERM ESSENTIAL POLICY allows you to enjoy life knowing you have coverage for injuries and sudden-onset illnesses.
MEDICAL COVERAGE FOR 30 TO 181 DAYS
This may be right for you if you are:
- Between jobs, laid off, or on strike
- Waiting to be covered under a group medical plan, or an individual policy
- A recent graduate, or taking a break from school
- Just starting a business
- In need of temporary medical insurance
Eligibility
You are eligible for coverage under this policy if you and any family members applying for coverage:
- Are under age 65 and will remain under age 65 for the term of the policy. Unmarried dependent children must be under age 23 and dependent upon you for support. Generally, the child must live with you. The exception is when you are legally required to pay for part of the child's support and there is no court order requiring that someone else provide insurance for the child
- Are not eligible for Medicare
- Are not pregnant. If any member of your family is pregnant, you may not apply for coverage until the pregnancy terminates
- Are not covered under any other hospital or medical plan
Temporary Coverage
SHORT TERM ESSENTIAL POLICY is specifically designed to provide medical coverage on a short term basis. Only one SHORT TERM ESSENTIAL POLICY may be purchased within any 12 month period. No policy may be reissued or renewed and SHORT TERM ESSENTIAL POLICY is not intended to replace permanent coverage.
Choice Of Providers
You may receive care from the physician or hospital of your choice; however, you may receive a higher level of benefits by using the IPN Network. Visit our website at www.primaryhealth.com or call our customer service department at (208) 333-1596 to verify that your provider is in the network. If you are out-of-state and need care, in-network providers can be accessed through our out-of-state travel program.
