Health Plan changes the benefit plans from time to time as required by law, as the
needs of our members change, or as necessary to keep pace with the benefit practices
of competitors. Therefore, this Handbook is provided for reference only and may
differ from employer group to employer group. Please call Customer Service to request
a member handbook that is specific to you and your employer group. A printable version
of the most current

Please note: the information in this section is a summary only designed for your
ease in using the Plan. It is not the plan legal document. If you would like a copy
of the plan legal document, please contact your employer or Primary Health Plan.
The plan legal document is called the Master Policy. The Master Policy supersedes
this Handbook. If there is any discrepancy between the Primary Health Plan
Member Handbook and the
Master Policy, the Master Policy will be the ruling authority.
